Curated Disease DAG, Not NCIt Hierarchy

The disease hierarchy exposed through canonical.disease_hierarchy,
q_disease_hierarchy, and q_diseases.descendant_keys is the versioned Bioloupe product DAG.
NCIt validates disease identity and can suggest review candidates, but the full NCIt hierarchy is
not imported as the product rollup hierarchy.
Considered Options
- Import the full NCIt parent hierarchy.
- Rebuild hierarchy only from source mentions.
- Recover the initial product DAG from legacy product intent, issue it as a reviewed Bioloupe release, and maintain later versions through explicit review.
Consequences
Product search and analytics rollups keep the intended grouping behavior without a runtime legacy dependency. NCIt remains the concept identity authority rather than the rollup authority; ADR-0010 defines release ownership and immutability.
